Authorized Source Of Information

The only source of authorized information is the benefit booklet and booklet inserts, if any, the Trust Agreement, the Rules and Regulations and the written statements of the Board of Trustees and its authorized agents located in Downey, California. Statements or representations made by individuals other than these designated personnel are not authoritative sources of information.

Questions as to eligibility, benefits and other matters should be submitted to the Fund at its office at 9901 Paramount Boulevard, Suite 211, Downey, California 90240.

Plan Amendments and Implementation

Any modification or amendment to the plan of benefits described in the health and welfare booklet must be in writing and adopted by the Board of Trustees. Only the Board of Trustees is authorized to interpret the Plan of benefits described in the benefit booklet. The Board’s interpretation of the Plan of benefits described in the booklet shall be final and binding on all parties. No Employer or local Union, not any representative of any employer or Union is authorized to interpret the Plan on behalf of the Board, nor can such person act as an agent of the Board of Trustees.

Nothing in this statement is meant to interpret or extend or change in any way the provisions expressed in the Plan. The Trustees reserve the right to amend, modify or discontinue all or part of this Plan whenever, in their judgment, conditions so warrant.
